Initial Evaluation
We will pair you up with one of our excellent providers to come to you and perform an initial evaluation to determine the details and next steps of your treatment.
Insurance Verification
Once we have determined what your needs are, we will need a picture of your ID and insurance cards sent to us. Your insurance will be run to ensure coverage for your care needs.
